  • Virtual/Phone Consultation

    To get started, please review our fees and policies, then complete the webform on our website to request a free phone consultation. During this initial call, we'll discuss your primary concerns, symptoms, treatment goals, and answer any questions you may have. If we determine that our services are a good match for your needs, we can discuss scheduling your initial evaluation.

    Please be aware that we are not contracted with any insurance carriers and do not accept Medicaid (Health First Colorado). We can provide documentation for you to submit to your insurance for potential reimbursement.

  • Psychiatric Assessment

    The psychiatric assessment is dedicated to the thorough evaluation of your current symptoms and a comprehensive review of your mental health, medical, social, educational, substance use, and family mental health histories. This process is designed to clarify your current condition while identifying any other medical or psychiatric factors that may be relevant to your care.

    For minor patients, the assessment will include meetings with parents/guardians to gather additional information.

    We understand that discussing certain aspects of your history may be difficult, especially for individuals with trauma. You always have the right to decline discussing any information for any reason. We will address this at the beginning of your assessment and invite you to identify any topics you prefer to skip. Please note that certain information may be necessary to arrive at an accurate diagnosis or develop an appropriate treatment plan.

    Important: Medications are not prescribed during the initial assessment.

    Treatment Planning

    After completing your assessment, we'll schedule a treatment planning appointment to:

    • Review diagnoses and treatment goals

    • Discuss evidence-based recommendations for your care

    • Create a personalized treatment plan

    • If indicated, discuss medication options, obtain informed consent, and prescribe treatment

    This appointment is particularly important for:

    • Complex psychiatric presentations

    • Patients receiving both psychiatric medication management and ketamine therapy

    • Cases requiring careful coordination of multiple treatment approaches

    Our approach considers multiple evidence-based interventions, which may include:

    • Medication management

    • Nutritional psychiatry

    • Psychotherapeutic modalities

    • Ketamine therapy protocols and integration

    • Lifestyle modifications and personal health habits

    You are always in charge of your treatment decisions. We will provide information and recommendations, but the final decisions about your care remain yours.

  • Fee Schedule

    • Initial Psychiatric Evaluation: $450 (90 minutes)

    • Treatment Planning: $400 (50 minutes)

    • Follow-up Appointments:

      • 25-minute appointment: $200

      • 50-minute appointment: $400

    • Ketamine Treatment:

      • In-office IM ketamine session (90 minutes): $450

      • In-office IV ketamine session (90 minutes): $550

      • Additional time beyond 90 minutes: $300 per hour (billed in 15-minute increments)

    Note: For existing patients receiving IV ketamine therapy, a transition period may apply before adjusting to current pricing. Please discuss with your provider for details.

    Note: Patients receiving both psychiatric medication management and ketamine therapy may require more frequent follow-up appointments initially to ensure proper monitoring of both treatments.

    No Surprise Act and Good Faith Estimate

    The frequency of appointments is clinically determined and affected by your diagnosis, adherence to treatment, life stressors, emergencies, engagement in therapy, self-care, and other factors.

    For Established Patients: Depending on your needs, your cost estimate for psychiatric medication management could range from $800 to $4,800 annually (based on quarterly to monthly 25-minute follow-up appointments).

    For New Patients: The estimated cost for a psychiatric evaluation is $450, with a subsequent treatment planning session of $400. The frequency and duration of follow-up appointments will be determined by your specific clinical needs, particularly if you are receiving both psychiatric medication management and ketamine therapy.

    Payment Policy

    • Payment is due at the time of service

    • We accept cash, credit cards, and HSA/FSA cards

    • If you have a balance on your account, you will receive a billing statement with a 30-day grace period

    • Failure to make payment within 30 days may result in late fees

    • If you are experiencing financial difficulties, please contact our billing department to discuss payment arrangements

    Administrative Work and Additional Services

    • Brief phone calls, emails, or texts are not billed

    • Clinical services requiring more than 15 minutes will be billed at $450 per hour in 15-minute increments

    • Other services such as completing forms, coordination of care, prior authorizations, court hearings, or transferring medical records will be billed at $450 per hour in 15-minute increments

  • Attendance

    We reserve time exclusively for you and ask that you make every effort to attend your scheduled appointments.

    Cancellation Policy

    • Appointments must be canceled at least 24 hours in advance to avoid charges

    • Cancellations with less than 24 hours' notice will be charged up to the full cost of the scheduled appointment

    • For Monday appointments, cancellations must be made by the same hour on the preceding Friday

    • Payment for cancellation fees must be made before your next appointment can be scheduled

    Late Arrivals

    If you are late for an appointment, we may need to reschedule, resulting in a charge of up to the full amount of the scheduled appointment.

    No-Shows and Repeated Cancellations

    • All cancellations and absences are documented in your medical record

    • Three instances of no-shows, cancellations, or late arrivals may result in adjustments to your appointment policy or discontinuation of care

    • After two months of inactivity, the treating relationship will be terminated

    Provider Cancellations

    In the event that your provider must cancel an appointment, no charge will occur, and we will notify you immediately. We will make every effort to reschedule your appointment at the earliest convenience.

    Appointment Reminders

    We offer appointment reminders via email and text. You will automatically receive reminders 24 and 48 hours before your appointment. You may opt out of these reminders at any time.

  • Treatment Approach

    Medications may be indicated when your mental health symptoms are not responsive to psychotherapy or other non-pharmacologic interventions. When mental illness significantly impacts your ability to work, maintain relationships, or care for your basic needs, medication may offer relief.

    If we agree that medication is appropriate, we will:

    • Discuss available medication options for your condition

    • Provide information about how the medication works

    • Explain dosage, frequency, expected benefits, and possible side effects

    • Discuss potential drug interactions and withdrawal effects

    • Ensure you have all necessary information to make an informed decision

    Controlled Substances Policies

    Benzodiazepines and Anxiety Treatment

    Due to abundant evidence that benzodiazepines (Xanax, Klonopin, Ativan, Valium, and others) can worsen depression and anxiety, disrupt sleep architecture, interfere with learning and memory consolidation, impede recovery from trauma, and place patients at higher risk for abuse and dependency, they are not offered as a standard treatment for sleep, trauma, or anxiety at The Untitled Mind.

    While we do not typically prescribe these medications for ongoing symptom management, we regularly work with patients to:

    • Find effective alternatives to benzodiazepines

    • Develop individualized tapering plans to slowly and safely reduce and discontinue benzodiazepines

    • Provide support throughout the transition to more sustainable treatment approaches

    Benzodiazepines may be appropriately used in specific, time-limited circumstances such as:

    • Directed treatment for specific conditions

    • Substance withdrawal protocols

    • Acute crisis management

    If you are currently taking benzodiazepines and interested in assistance with safely discontinuing them, please discuss this during your assessment.

    ADHD Treatment Policies

    The Untitled Mind maintains a conservative approach to prescribing stimulants, guided by the following policies:

    • We do not prescribe stimulants to clients using cannabis on a regular basis, as cannabis is not supportive of cognitive function and can complicate ADHD treatment

    • Moderate to high intake of alcohol, use of recreational substances, and various controlled medications are typically contraindicated in managing ADHD

    • All patients receiving controlled prescriptions (including stimulants) will receive lab orders and urine drug screens periodically

    • To prevent tolerance and dependence, stimulants are prescribed for work and/or school days and not intended for daily use

    • Adult patients are recommended to complete psychoeducational testing prior to starting medication management

    • While screening instruments like the Conners Test are useful tools, they do not have the sensitivity and specificity of comprehensive psychoeducational testing

    • Collateral information may be requested during the assessment process or ongoing treatment

    Ongoing Medication Management

    • If medications are prescribed or changed, a 15-30 minute follow-up will be scheduled within 2-4 weeks

    • As symptoms improve, follow-up visits may occur at monthly intervals

    • For patients on stable maintenance therapy, follow-up visits may be scheduled at three-month intervals

    • Alternative treatment structures may be considered based on individual circumstances

    Coordination of Care

    If you are receiving psychotherapy from another provider, we will coordinate care with your therapist with your written consent. Communication between mental health professionals is key to providing effective treatment.

    Second Opinions

    If at any point during your care you would like a second opinion, you are encouraged to do so. We can assist in recommending another provider to review your treatment plan.

    Refills

    • Medications are refilled during scheduled appointments with enough supply to last until your next appointment

    • If you need a refill between appointments, please contact our office directly

    • Pharmacy refill requests are often automated and may not reflect your actual needs

    • Refills without an appointment are discouraged and may incur a $100 charge

    • Please plan ahead to have an appointment scheduled before you need a refill

    Pregnancy and Medication

    Pregnancy or attempting to become pregnant while on psychiatric medications has risks up to and including death of the fetus and/or mother. Most medications are contraindicated during pregnancy or when trying to conceive.

    If you are pregnant or planning to become pregnant, you must inform your provider. If we provide care during pregnancy, you must also have an OBGYN with whom we will consult before prescribing or changing any medications.

  • Overview

    Telemedicine allows us to provide psychiatric services using interactive audio and visual electronic systems when you and your provider are not in the same physical location. Our systems incorporate security protocols to protect the confidentiality of your information.

    Requirements for Telemedicine Appointments

    • You must be a resident of Colorado and physically located in Colorado during appointments

    • You must have a device with a camera and microphone

    • You need internet access with a download speed of at least 10 Mbps

    • You must be in a private, quiet location

    • You must be sober, clothed, and not driving during appointments

    Technical Setup

    • A secure link will be sent in your appointment reminder email

    • For computers, the link will open in a new browser tab

    • For phones or tablets, you'll need to download the telehealth app

    • Test the link before your appointment

    • Use headphones for better audio quality and privacy

    • Close other applications and silence notifications to minimize distractions

    • Consider using a white noise app near the door for added privacy

    Potential Benefits

    • Increased accessibility to psychiatric care

    • Improved continuity of care

    • Greater convenience

    Potential Limitations

    • Technical issues may affect the quality of the session

    • Your provider may not be able to provide emergency care remotely

    • Equipment failures may cause delays in evaluation and treatment

    • Security protocols may be more vulnerable to privacy breaches

    • Limited visual information compared to in-person visits

    Client Rights and Responsibilities

    • Laws protecting privacy and confidentiality apply to telemedicine

    • You have the right to withhold or withdraw consent for telemedicine at any time

    • Your provider may withhold or withdraw consent for telemedicine if clinically indicated

    • You may not record sessions without written consent from your provider

    • You must inform your provider if anyone else can hear or see the session

    • You are responsible for the proper functioning of your equipment

    Emergencies

    If you or someone you know is experiencing a medical or psychiatric emergency, please call 911 or go to the nearest emergency room immediately.

  • Maintenance of Records

    We are required by law to maintain complete medical records. Your records are stored electronically in a secure system. Any written records will be kept in a secure location.

    Access to Records

    • You have the right to request copies of your medical records

    • Records can be made available to other healthcare providers with your written consent

    • You may review your records unless we determine that doing so might jeopardize your well-being

    • We recommend reviewing records with your provider to minimize misinterpretation

    Fees for Records

    Time spent collecting, printing, copying, and summarizing medical records will be billed at our standard rates. If records are required for court documentation, copies will only be provided with a court-ordered subpoena.

  • Protection of Information

    The security of your sensitive information is of utmost importance. We are bound by law to protect your confidentiality and follow HIPAA regulations. Any disclosure of your treatment to others will require your explicit written consent.

    Recording of any kind (audio, video) during sessions, within our premises, on the phone, via video conference, or during any form of communication is strictly prohibited.

    Exceptions to Confidentiality

    There are situations where disclosure is mandatory:

    • If you pose a threat to the safety of others

    • If you are at risk of harming yourself

    • If required by a judge in legal proceedings

    • If cognitive dysfunction prevents you from safely operating a motor vehicle

    • If mental illness prevents you from providing for your basic needs

    In these situations, we will make every effort to discuss the situation with you before taking action.

    Access to Records

    • You have the right to request copies of your medical records

    • Records can be made available to other healthcare providers with your written consent

    • You may review your records unless we determine that doing so might jeopardize your well-being

    • We recommend reviewing records with your provider to minimize misinterpretation

    Fees for Records

  • Right to Refuse Treatment

    You have the right to refuse treatment. If you do so, we will explain the nature, purpose, risks, and benefits of the proposed treatment, as well as alternatives and the consequences of not proceeding. You will have the opportunity to ask questions and have them answered satisfactorily.

    If you refuse treatment, you release the practice, its employees, and your provider from liability for effects resulting from your decision.

    Patient Rights and Grievance Policy

    If you have concerns about your care, we want to address them promptly. To express a grievance:

    We aim to resolve all concerns in a timely and efficient manner.

    Discontinuation of Care

    Our providers reserve the right to discontinue care if:

    • You become non-compliant with treatment

    • You behave in a belligerent or disrespectful manner

    • You pose a danger to our staff

    • Your needs would be better served by another provider

    If you choose to discontinue care with us, we request that you complete an exit interview to discuss proper medication discontinuation methods and alternative treatment options.

  • Insurance Policy

    We do not accept insurance and are not contracted with any insurance carriers. Payment is required at the time of service.

    Superbills for Reimbursement

    Upon request, we can provide a superbill (statement) that you can submit to your insurance company for potential reimbursement. This statement will include:

    • Date of service

    • CPT codes

    • Diagnosis codes

    • Provider information

    We do not guarantee reimbursement by your insurance company and do not communicate with insurance companies on your behalf.

    Billing Codes

    For insurance submission purposes, we use the following codes:

    • Initial evaluation: 90792

    • Treatment planning: 99215

    • Standard follow-up: 99214

    Collections

    If a payment is 90 days past due, we reserve the right to use collection agencies or small claims courts to obtain payment. If your account is submitted to collections, a 40% fee will be added to your outstanding balance.

  • Parents, Spouses, and Loved Ones

    For Parents of Minors: Colorado law states that children 15 years and older hold the right to consent for and terminate treatment. To start treatment, prescribe medications, or terminate care, we must have the patient's informed consent.

    For Parents, Spouses, and Loved Ones of Adults: HIPAA protections prevent us from sharing information without the patient's consent. If you wish to be involved in treatment or receive information, please ask the patient to sign a release of information with our clinic.

    Forms and Letters

    We complete forms and write letters for established patients when clinically indicated. Please note that we do not complete state disability forms. If you need documentation for accommodations or leave from school or work, please keep us informed of symptoms and functional impairments in real time.

    Practice Relationship

    All patients must have an appointment scheduled at all times if they are in treatment with The Untitled Mind. After a no-show or cancellation, we will reach out to discuss scheduling and your interest in continuing treatment. After two months of inactivity, the treating relationship will be terminated, and we will mail a termination letter via certified mail.
